Key points
- ≥5% weight loss cuts liver fat. 7–10% improves MASH. ≥10% can reverse fibrosis.
- Mediterranean-style diet. Cut sugary drinks and ultra-processed food.
- 150–300 min/week moderate activity (or 75–150 vigorous), plus resistance training.
- No alcohol if there is advanced fibrosis. Stop smoking.
For the evidence, graded recommendations, a case and self-test questions, read Module 3: The lifestyle prescription.
1Weight loss and histology
In a prospective study of 293 patients with biopsy-proven NASH, those who lost ≥10% of body weight had NASH resolution in 90% and fibrosis regression in 45%. Even 5% loss led to NASH resolution in 58%.
2Diet
- Calorie deficit of about 500–1000 kcal/day for weight loss.
- Mediterranean pattern: vegetables, legumes, whole grains, olive oil, fish, nuts.
- Limit fructose-sweetened drinks, refined carbohydrates, red and processed meat.
- Coffee is linked to lower fibrosis risk in observational studies.
3Activity
Exercise lowers liver fat even without weight loss. Pick what the patient will keep doing. Add resistance training to protect muscle, especially with GLP-1 drugs and in older adults.
4Alcohol
Alcohol and metabolic risk add up. Advise complete abstinence in advanced fibrosis or cirrhosis, and as little as possible otherwise.
